Our Dental Savers Plan was designed with all our patients in mind. Especially, patients that don’t have insurance, don’t like
their dental insurance, patients that need a lot of dental work, or patients that just want to save money.
$199 per year per adult (Ages 14+)
$125 per year per child
Includes:
- 1 Comprehensive exam
- 1 Comprehensive periodontal evaluation
- 1 Cleaning (In the absence of periodontal disease)
- 1 Panoramic radiograph
- 2 or 4 bitewing radiographs
- 1 Emergency exam (During normal operating hours)
- 1 Periodic exam

saving you more money even after the first appointment.
Coverage:
- 100% off all necessary X-rays
- 15% off all procedures (Excluding Orthodontics, Oral-B products, Burst products, and office
dispensed adjuncts)
- 10% off Invisalign and Orthodontics
- $20 per sealant; $20 fluoride varnish

Important Details:
- Benefits are effective immediately upon enrollment and are effective for 1 (ONE) calendar year from the date of enrollment.
- Lyon Family Dental Savers Plan cannot be used for dental care outside of Lyon Family Dental, PLLC.
- Lyon Family Dental Savers Plan is NOT a form of dental insurance. It is a membership plan to assist our patients and their families to obtain the dental care then need at an affordable price.
- Cannot be combined with other offers, dental plans, or insurance plans.
- Patient portion of the bill is due at the time of service.
- Patient has 30 calendar days after plan start date to opt out. If patient opts out of plan within 30 days, the full premium will be refunded, and the full cost (including previously applied discounts) of any procedures performed will be due.
- After 30 calendar days, NO refund of premium will be provided.
- No membership card is provided. Membership information will be kept on file with Lyon Family Dental, PLLC.
- Plan cannot be used for injuries covered under worker’s compensation or automobile medical insurance.
- Office fee schedule is subject to change.
- Plan cannot be used for treatment for which, in the sole opinion of our providers, lies outside of the realm of their capacity.
